I acknowledge that I have read the RULES OF PROCEDURE OF THE CAIXABANK SHAREHOLDER ADVISORY COMMITTEE, and I accept them in their entirety. If my application for membership of the Shareholder Advisory Committee is successful:

• I agree to sign a confidentiality agreement as to any information to which I may gain access in the course of my involvement with the Shareholder Advisory Committee.

• I disinterestedly authorise CaixaBank, with worldwide effect and for an unlimited time, to disclose and publish my name, city of residence, occupation and image, particularly in the following media:

• website, in-house newsletters, annual report and, in general, any other means of communication relating to CaixaBank's financial reporting or aimed at CaixaBank shareholders as a class

• any other medium, whether known or unknown at the present date and whether in existence now or in the future (electronic, optical, numeric, computer-based, etc.)

• I undertake to retain the ownership of at least 1,000 CaixaBank shares throughout the duration of my duties as a member of the Shareholder Advisory Committee.

Your data will be processed for the purpose of sending you the information you request from us. We may also use your data to send you corporate information and news on the events that we organise, provided that you have granted your consent to this. Your data will also be processed in order to comply with regulatory requirements and obligations.

This data can be communicated to public authorities, for the compliance of a required legal obligation, as well as to service suppliers and to third parties necessary for the management and execution of this application.

The data will be processed while the duration derived from the request in force; once the validity has expired, the data will be stored (during the limitation period of the actions derived from these relationships) for the sole purpose of complying with the required legal obligations, and for the formulation, exercise or defense of claims.

Exercise of rights and claims before the Data Protection Authority

The owner of the data will be able to exercise their rights with regard to your personal data in accordance with the current regulations, in CaixaBank's offices, at the P.O.B. 209-46080 VALENCIA or at www.caixabank.com/exerciseofrights , indicating the reference "SHAREHOLDERS Data Protection Right". Given the special nature of such information, if you make a generic rights request to CaixaBank, S.A., this information will not be reflected.

Likewise, you can direct claims derived from the processing of your personal data to the Spanish Data Protection Authority (www.aepd.es ).
